Request for Medical Records:

If you are looking to send and/or receive your medical records to/from our office, please email us at contact@crofootmd.com or call us at 713-526-0005 so we can send you a secured request form via Hushmail.

Patient Responsibilities:

You have the responsibility to participate in decisions regarding your own care and to clarify the consequences of a decision to refuse care. You are responsible for any consequences or adverse effects you may incur as a result of refusing care or not complying with instructions given to you. You have the responsibility to provide a complete and accurate health history and to tell your provider if you have any changes in medication, procedures and/or your physical condition.

Insurance and Patient Fees:

Gordon E. Crofoot MD, PA participates with the vast majority of insurance plans, including Medicare. Our knowledgeable and friendly staff will help you understand if our services are covered and if you will be responsible for any unmet deductible, coinsurance or co-pay amounts. These amounts, as well as any service provided by Gordon E. Crofoot MD, PA that is not covered by your health plan, will be payable at the time of your visit. Please bring your insurance card to every visit so we may verify your medical coverage.

*If you have any additional questions regarding fees or are in need of making special payment arrangements, please speak with our office administrator. As a courtesy to you, our office will file insurance claims. However, patients assume the ultimate financial responsibility for payment.

After-Hours Assistance:

CrofootMD is always on-call when the office is closed. If you have an urgent problem, please keep in mind the following:

-Routine medication refills are only filled during office hours

-If you have a life-threatening or medical emergency, please call 911 or go to the nearest emergency medical facility

-In order to reach someone after-hours, please contact our office at (713) 526-0005

Details About Our Location:

Our Clinic, Research Center, LabCorp, and Walgreens Pharmacy are all located within our office to provide collaborated patient-focused care.

We are located at 3701 Kirby Drive in Houston, TX 77098. Our building offers free visitor parking in the first-floor garage or street parking surrounding the building. Upon entering the building on the first floor, you can take the elevators to the twelfth floor. When you exit the elevator, you will turn towards the double glass doors. We are located down the hallway to the left of those double glass doors in suite 1230.

Public restrooms are located just outside our suite in the hallway.
